Making Accounts

I Need Help making accounts i made my own and i tryed copying it and pasteing it and making it not admin or gm or counseler but when i tryed the account it said it wasnt found can someone please show me a layout of the accounts and how to make em?

Dont try editing accounts.txt manually. You'll only cause a ton of problems doing that.
